The camera lens cover was replaced, it's just the small glass piece that goes over the camera to protect it. It was just cracked and I replaced it very easily. No new parts are on the phone and there's no pop up messages

Just one more clarification needed: do you have any idea if the phone was opened or disassembled entirely when the camera lens was replaced? Have you replaced it in Apple store or any third party service?

phone has never been opened or disassembled at all. Here's a link to a replacement kit for your reference, it's a very simple remove and replace on the outside of the phone that I just did myself. I've done it on previous phones without issue.
