T-Mobile Note 2 minus charger

I loved this Note 2, but after my workplace gave me a similar phone on a company plan, I eventually cancelled the data line /sim entirely. It has some barely discernable scratches on the screen, and a couple cracks in the back cover that have no effect on functionality. It always ran great and I miss using the s-pen (which is included, of course). Only thing really missing is the charger, the cable was so worn out that it barely worked, and it was misplaced.

Is the screen original? Is it unlocked?

The screen is original - the device was purchased new at Best Buy Mobile and has never needed repairs. At this time it is not unlocked, so I would expect it to work with T-Mobile only. I believe I could get it unlocked, but I'm not really sure where I would need to take it to get it done.
