@Jim K. - No, the screen and battery have never been replaced. The phone has never had any repairs. I purchased it brand new from Best Buy in March of 2017, so the 1-year warranty expired in March of this year.

Is this phone unlocked on GSM and CDMA. Will it work with a verizon sim card and a tmobile sim card?

I can confirm, because I've used this phone with Verizon, T-mobile, and currently Sprint. All without issue.
