Hi Falcon,
I uploaded two good pictures. It is as best I can tell without any physical damage. There is a small amount of residue/glue which I removed (around where the screen protector has been).

It is not unlocked. It can be used with AT&T or H20. I knocked a couple dollars off the price but it was already a pretty good deal.
