what is the battery health check status of this phone?
Is this an unlock T-mobile phone which I can use on other carrier?

Battery is at 100, it's 12 months old. I bought from Samsung and never was locked. According to the networks it will support, it shows all. I just got it as t-mobile because thinking updates quicker. I did try it on Spectrum and that is CDMA while I was on GSM.
