Battery Health is 96% (Replaced with genuine apple battery at apple store in Brandon, FL in March 2019)

The iPhone model is A1633, please make sure that you can use on your carrier before purchase. This was a model originally only sold by AT&T, but google search says it can be used on AT&T, Verizon, T-Mobile and MVNOs like Visible (Verizon network), Mint Mobile (T-Mobile network) and Cricket Wireless (AT&T network).
