Includes everything it originally comes with, and I've used it with a hydrogel screen protector on top of the preinstalled screen protector.
I've used it with Mint with no issues.

What happened to corner? Drop damage?
@Matt, Not actually sure what exactly happened, but not a drop for sure. Just saw it one day a month or so back while I was wiping the phone down.
Weird. Do you have more detailed pictures of it and all trim? Some of these are a little blurry and hard to tell detail.
