Is it eligible to be unlocked for T-Mobile?
It is not eligible. Not for another 37 days even though it was purchased and activated on over 40 days ago. The line that the phone was connected to ported out before the 40 days. So T-Mobile restarted the unlock countdown on their network.

1. Is the phone fully paid off and not tied to any carrier balance?
2. Has it ever been reported lost, stolen, or blacklisted?
3. Is the iCloud account signed out?
4. Is there something wrong with the phone?

The device is fully paid off and has no balance tied to it.
The device has a clean IMEI, it is not reported lost, stolen or blacklisted. (I believe any of this would disqualify it from being listed on swappa)
There is no activation lock, iCloud account is singed out.
There is nothing wrong with the device.
There is only a few days of usage and battery cycles on it. It is locked to T-Mobile for 40 days of consecutive usage before they will unlock it. The number on my account that bought the device ported out before those 40 days, so I have to wait about another month to unlock it.
