In perfect working condition with no damage, cosmetic or otherwise. Includes Apple magnetic charger and 2 bands - the sport loop is Apple brand and the metal band is a third party. (The bands do have some visible wear.)

Unfortunately not. However if you are planning to activate it with Verizon they are doing open enrollment for their extended warranty/insurance through Nov 3rd so you could add that for protection.
