It's a Verizon phone so I don't know if you can change it to prepaid or not. There is absolutely nothing wrong with the phone. It works flawlessly.

Are there any scratches on the screen and do you know how many months are left on the warranty and what the battery health of the phone is?
No Scratches on the screen.
Not sure of the warranty. I've had it for around a month so it should be good for a while.
Battery is excellent. Will last multiple days with light use and all day with heavy use.
Is the phone unlocked?
Yes, it's unlocked. Verizon unlocks all of their phones now. (except prepay phones I think)
