A factory unlocked iPhone was sold by manufacturer as unlocked. It has never been locked to a mobile network.
Carrier unlocked iPhone was sold by a network company such as Verizon and AT&T and thus can locked to that carrier.

Not sure about every carrier around the world. I can only assume the carriers here in the US.
