Sorry for the late response. I am the original owner. It's been a great phone. I had the battery replaced by Apple a couple years ago as it was one on the recall list. I just replaced the charging port and that works great now.

Yes, I do have the receipt for the battery. Our IT director at work helped find a repairman with the charging port.

Yes, I always had a case and screen protector on. I actually still have them on the phone. The battery life shows 88%.
