All functions work. Phone has no cracks. Phone has some marks and wear in my opinion. Few nicks/dings on sides and back. Please check the pictures and determine if the cosmetic condition is up to your standards. We took a large amount of pictures so you can see all angles of the phone. Screen has been replaced recently with a non oem(Not apple) screen, and it works great.

It will remain on iOS 11. Yes works great on T-Mobile. Tested with my sim.
What the battery health? If this iOS version doesn't have the feature you can download a app called Battery Life to determine the wear level.
