The phone is in very good condition. It was always kept in a case with a screen protector.
The screen has no scratches (see photographs). The body has no scratches other than some in the shape of an oval that were created by the protective case opening for the camera lens and flash (no scratches on the lens itself). The phone has never been jailbroken.
No cable, charger or headphones are included (i.e. phone only). It will be shipped in a generic box.

I'm not sure if it can be used on another carrier. I can tell you it was purchased through T-Mobile.
