The iOS version is whatever the most recent one is. Warranty up 10/6/19.

No brand new from apple about 1 month ago. The prior phone had a malfunction so they did a full replacement.

Yes was required to unlock it. I am uploading screenshot from T-Mobile app now showing paid off as verification.
So, is this phone unlocked to be used with any carrier or t-mobile locked?
It is carrier unlocked however I have only tested on AT&T. I am not promising it will work on any network but T-Mobile since I don't have access to those Sim cards from those networks to test. If you buy it for another network and it doesn't work, I will not be responsible for that. I have attached a photo showing att working using my work sim card.
