Is this locked to verizon and is the scratch on the actual screen?

The scratch is on the actual screen but very light and visible at certain angles. It is only noticeable once you take off the screen protector.

This phone is unlocked — Verizon automatically unlocks phones after 60 days. I’ve had it for over a year.
