This is a Samsung Galaxy S5 for Verizon that is in near mint condition. The screen of the phone is in mint condition and looks amazing. The edges and bezel around the glass have two small dings on the bottom of the phone and the charging port cover has a paint chip. The back of the phone looks amazing and is in mint condition too. Hi Raul,

It does not come with the SIM card. We cannot provide them because they are a one time use thing and need to be set up for your account. Verizon, or any of the major carriers, will give you a SIM card for free though and set it up for you.
