My mother was the primary user. She doesn't use her phone much other than to answer calls and text. It was always stored in her purse. Because of her light and careful usage, the phone is in excellent condition. No scratches on the screen. One tiny scratch or something on the corner. It's about 2mm long. See picture.
It has been wiped clean and ready for a new user to setup.

I got the phone today and typed in *#06# per Cricket's instructions and got no MMI. The phone is unlocked, isn't it? There is a lock symbol on the screen along with T-Mobile. How do I get the IMEI number for the phone?

The phone is carrier unlocked like all T-Mobile phones.
The lock symbol at the bottom of the screen during boot up indicates the Bootloader is locked. Bootloader is the low-level software on your phone that keeps you from breaking it. It is used to check and verify the software running on your phone before it loads. It has nothing to do with carrier unlock.
The IMEI number is on the box. You can also find it after you setup your Moto X by going to Settings > About Phone. It is somewhere in there.
