The device is in mint conditions and shows mimum usage. The front screen shows very little signs of usage and has no visible scratches or blemishes, the middle frame is in like new conditions too, there is a small ding on the frame at the top right corner and lastly the back cover is in great conditions as well and has been replaced with a blue/titanium grey T-mobile back cover.Everything has been tested to work well on the device. Is that phone refurbished? It is listed as mint, but using the back cover from different phone color?

Yes as mentioned in the description, the plastic frame and back cover have been replaced with t-mobile note 2 and all parts are original. They were replaced as the tmobile note 2 was cosmetically in mint conditions but the logic board was bricked due to a software problem.
