32 GB Galaxy Nexus - Verizon - CyanogenMod 11 Installed
This phone is in perfect condition. I got it less than 6 months ago, and rooted it immediately. There are no scratches on the screen or any other cosmetic problems. I'll include the original AC adapter and USB cable, and the casemate "Barely There" case I used for it.
Seriously, this phone is in great condition. I take great care of my gadgets and it shows.
