Is this a us cellular phone? Locked or unlocked?
Will it work with T-Mobile ?
Per US Cellular all devices are unlocked.
All 4G LTE devices launched by U.S. Cellular on or after February 1, 2016 are sold unlocked.

I am not positive about T-Mobile. I tried running their device compatibility / IMEI check but no results came back.
Correction - It does show compatible with T-mobile:
