I apologize. I think I understand the Swappa procedures now. Here are a few questions:
- When was the phone purchased?
- Are you the original owner?
- Is this phone a replacement?
- When was the phone activated and when does the original warranty expire?
- If it was a replacement, was it replaced at Apple?
- Is the phone fully paid for, or is there a financing arrangement?
- I realize it is not required, but is a SIM card included? My NEWEST phone is an iPhone4 (!) which does not have a SIM.

Thanks for your interest in this phone. Let me do my best to answer your questions.

I don't remember exactly when I bought it, but I believe it was in November 2014.
I am the original owner and it was activated the day I bought it.
No idea when the original warranty expires, but I think it's only one year so it's likely long gone.
Phone is fully paid for,
I think it has a SIM in it, if I can find something to open the tray I will verify, but you should be able to go into whatever carrier you have and get a SIM if they took it out when I got my new phone.
