Comes with ONLY the phone and battery, if you need a micro USB 2.0 I can include that but they are pretty common, just let me know. I don't have the USB 3.0 that also works with the S5, which does have faster charging and transfers data faster.
Damage description
Nothing to report, phone works like new! There are tiny scratches on the screen but they are barely noticeable.

Below is a list of some of the most popular phones on the market that will work with Straight Talk SIM. Before we get to that, here’s the basic criteria for a phone to work on Straight Talk SIM Bring Your Own Phone:
(1) Phone must be a GSM or CDMA phone with one of the major carriers.
If your phone was under a regular contract with AT&T, Cingular, T-Mobile, Verizon, or Sprint it should work.
If your phone was originally purchased for a prepaid plan on carrier that is not AT&T, Cingular, T-Mobile, Verizon or Sprint – there is a chance it may work, if the phone is unlocked. However, it may not work. Consider purchasing a low-cost phone from Straight Talk if you are in this situation.
(2) Phone must be either unlocked, or locked to AT&T, locked to Verizon, locked to Sprint, or locked to T-Mobile.
So yes, I believe it should work!
No dents or anything like that, looks like new. If you look carefully there are scuff marks on the screen from a general 2-3months of use but they are hardly noticeable, there are no major scratches/gashes.
