I had this phone for a little over a year and took great care of it. It was never removed from its Otterbox Defender case so the only cosmetic issues are around the charging port (from where I would lightly scratch it when putting the charger in blindly). There is also a verrrrry tiny nick on the left side. The screen is in original condition. It has never been cracked or replaced and there are no scratches. As far as the actual phone, it works flawlessly! I never had a problem with it, I just wanted a bigger screen. There is no sim card in the phone (I transferred it to my new phone) so you will have to use the one from your current phone or purchase a new one. I will also include a new Otterbox Commuter case that was used for about a week before I upgraded. It is in mint condition and still has the screen and applicator that have never been used.
