32GB US Red Nexus 5
This phone has been my daily driver for a while now, and still functions great despite damage that it has endured. I tried to capture the damage as best as I could, so please check out the photos, and let me know if you need more information before you decide to purchase.
Accessories included is one case.

Damage description
This Nexus 5 has some of the typical cracks that have been seen on many other ones including the one near the volume buttons, and SIM card tray, but not near the headphone port. No scratches on the camera lens glass. Scuffs around the front edges seem to be wear from the case. Again, please see the pictures and let me know if you'd like any more information.
Other than that, there are light scratches on the screen which are pretty much just visible when you're looking for them. I was able to capture it in the right lighting with a macro lens. There are also some normal wear and tear scuffing on the back.

@Hiren: The scratches are on the screen itself. The photos were taken with a macro lens to show the detail.
