I have added a verification photo as well as a photo showing the originally packaged charger, charging cable and owners manual. The box for this phone doesn't come sealed, but rather with the Nexus sleeve over the box which is also included in the pictures.

This is the Verizon version with the Verizon logo on the back?

Yes it came from Verizon. So it has the logo on the bottom of the back.
