UNLOCKED! Mint condition, and all factory-wrapped accessories / docs included.
This had a screen protector (now removed) since the minute it came out of the box. Absolutely no scratches, dings, or discoloration. You'll have to use a magnifying glass to find any cosmetic blemishes! All of the original accessories (including the AC adapter) are still in their factory wrapping, and you'll get the original box with original packaging.
I unlocked this phone to work with my AT&T SIM and custom firmware. I've reverted the phone back to the latest stock T-Mobile firmware, and the phone boots without any unusual messages or warnings. Although I used it as an A&T phone, as you can see, the ESN is clean for T-Mobile, too.

I have 5 questions.
1) Are you the original owner.
2) What is the manufactured date on the box.
3) Any repairs done on it like replacing the screen etc.
4) How would you rate the overall cosmetic condition of the phone 1 through 10.
5) Is this currently on an installment plan with T-Mobile or paid in full at the time of purchase.
1) Sort of? I purchased it as a new device on eBay about 7 - 8 months ago.
2) I'm not sure what you're talking about. There isn't one on the box. I also have the box from an AT&T version I purchased from an AT&T store. There's no date on that box, either.
3) No, none. It's all original, not refurbished.
4) 9.5. If you take a lighted 5x magnifying glass to the metal bezel around the camera, there are a couple of scratches on the metal.
5) Although I bought it on eBay, I did contact T-Mobile when it arrived, gave them the IMEI and confirmed that it was not on an installment plan, or attached to any account with a contract.
The manufactured date is usually on the sleeve that comes over the box looks like you don't have that.

I am a buyer at $375
The all new S6 will be unveiled on March 1 at MWC with a release likely in April 2015 that will bring the price of the new S5 TO $450 so a used for $375 it's a fair price.
If you can get T-Mobile to sell you an unlocked S5 without a new or existing account for $493, that's not a bad deal. If this is still listed in April, I'd be happy to reconsider your offer. But seeing as this has been listed for about 8 hours, I'm not in a hurry to come down over $100.
